The Sinclair Method recommends 50mg of Naltexone 1 hour prior to drinking alcohol. The 25mg recommendation you read in this forum is something we all have come up with to to ease a person into the 50mg dose as there are a few side effects (e.g., nausea). Naltrexone is a very safe drug. Worst case (if you take a full 50Mg), you might feel sleepy and/or like throwing up.
Suggest you start with 25mg for the first two days, then increase to 50mg after that. Remember to take Nal at least one hour before drinking. The first weeks, maybe you should work to taking Nal everyday and drinking at least a couple drinks one hour after taking Nal; everyday instead of binging, even though your pattern is to drink 2 days a week. Remember, extinction only takes place when you have Nal + Drinking. If you have a desire to binge, go for it, but only do it one hour after taking Nal.

This is an incorrect statement. I believe you are looking at reports made in this forum 1-3 weeks after starting the Sinclair Method, not the 4-6 months recommended in Dr. Eskapa's book. Indeed, most of us using the Sinclair Method experience a drastic decrease in drinking, and craving, the first 1-3 weeks after starting the Sinclair Method, but quickly "spike" back up to pre-Sinclair methods drinking levels after this. This experience is documented in Dr. Eskapa's book and verified by nearly everyone in this forum. We refer to this as the "honeymoon phase." It is short lived, but expected.
Remember: Nalrextone + Drinking = Cure (over time --- 3-6 months).
There are only a small number of people in this community who are at the 3-6 week mark because of how new this community is (i.e., it is at most 1 month old). Of these people who are 4+ months out, the results are very positive. In time, when more of us hit the 4-6 month mark, I have no doubt more positive first hand results will be reported.
If you do not have Dr. Eskapa's book, then you need to get it. Once you have this book, look at the extinction graphs made from the Clinical Trials. Take note that in the first few weeks of starting the Sinclair Method, drinking levels are very close to Pre-sinclair levels. The real results take place 3+ months out.
Extinction takes time. Naltrexone is not a get sober drug that will cure you in a couple days. This is a 4-6 month process.
