I started without reading the entire book, and I only read the medical part (what you give to your doctor). That was enough for me to get started. The book is a very is easy read (a little dry) that could be skimmed in about an hour, but it does help you in this process.
Please start with 1/4 of a pill to make sure you do not have any side effects. If you do not have any side effects on a 1/4, move up to 1/2 the next day. If you can please start to track your alcohol and try to get a feel on your cravings for alcohol and try to track that (for how will you know if it is a craving or just habit) there is a small part in the book that explains that. Please make sure you wait that hour (if not longer) and set a timer or an alarm on your cellphone.
Do not be surprised if you do not have desire to drink after taking that pill, for naltrexone is used for abstinence in therapy for alcoholic; however, it will not extinct your desire for drinking and that is not the process of TSM. Even if you do not want to drink, try to drink a little for even 1/4 of glass will help.
Try to drink that first and second drink as slow as possible (small sips not gulps). I try to make a drink last at least 30 minutes if not more. As you are drinking try to feel how your mind is responding to that drink. If you do this every time you drink, it will help you in the future.

http://www.webmd.com/drugs/2/drug-7399/ ... ideeffects). I am assuming that you are not on any opiates for taking naltrexone may cause mild opiate withdrawal symptoms.
I do hope that you will not be taking that first dose alone, just incase there are side effects. It is nice to have someone with you. I would also tell the person on you contact information and/or keep something in your purse next to your i.d. letting people know that you are on naltrexone incase of an accident.
