Hi all

I'm looking for some information please. My doc is claiming that from what's published the period of treatment is 3 months. I have tried to explain that TSM is more likely to take six or even upto one year and beyond. Even after this period I cannot drink without taking nal 1 hour before.

I cannot find something that's really to the point, to be honest a lot of stuff is a bit ambiguous.

I looked at the video recently put on here by Cheeto and it has Dr Sinclair in a bar explaining TSM. it very unprofessional and just won't cut the mustard with my doctor.

I'm really keen to stay with my GP as by and large we have a relationship that's positive. I just need to be in a better position to articulate in a way that's quick and fast to understand.

Hi thistime: Here is a post from Heavy Fuel from years ago that was Cured from TSM that is important to me. Not to burst your bubble but I would rather think long term and my Cured state happen early than to think 3 months and it does not happen in that time frame?

For those fearing the treatment isn’t working, or their drinking remains unchanged after many months, once any potential compliance issues have been resolved, at what point would you suggest that they may be one of the very small amount of people for which Naltrexone or Nalmefene does not work?

Is there any evidence to suggest a limit at which point the medication should be discontinued and other potential options explored?

Dr E: Probably, if after 10 or 11 months there is absolutely no benefit then the person may not be addicted through the opioid system … and could then try a traditional treatment.

I took my GP the book and showed her the chapter 'for physicians', she kept it and didn't give it back yet!! I think that has it written pretty clearly the needing to take the med for rest of life. She too was a bit taken aback, but I have explained that whilst my numbers aren't too much down I do feel my feeling re drink are changing.

to be honest she also hasn't gone ahead and prescribed me nalmefene I have to stick with what I have brought online and will continue to do so... in the long term it will be cheaper than drinking, I don't get any side effects, she has given me her blessing.. so ill continue. Had she had a huge problem I would have had a rethink, but if the hassle is just over the how long you take it for, with the knowledge of how low the side effects are both short and long term.. I would just live with it.

Is that the one where he gives the two minute explanation? If so, I agree, not very professional.

Honestly, I don't think clinical studies have investigated the full range of response time, and in practice, the anecdotal experiences of individuals who have applied TSM should not be dismissed. It sounds like your doctor is confusing clinical trial time frames with actual success or lack of success time frames, and they are not the same thing. The first proves or disproves efficacy, and the second is based on real world application and will include variable such as strength of addiction, duration of addiction, and genetic differences between individuals.

thistime, here's a 9 month time frame reference specifically for prescribing naltrexone for TSM. It's written by Stephen Cox, M.D., Asst. Clinical Professor of Psychiatry, University of Kentucky College of Medicine.

All your Doc has to do is pick up the phone if he wants to speak with Cox directly. (859) 323-6021 is the number for the U of K Psychiatry Outpatient Clinic, they should be able to point your Doc in the right direction.

Thank you so much for this information, it's just what my GP Is likely to pay attention to. The ability to give him a telephone number to call is mind blowing.

Just as a heads up I didn't test the number, just looked it up on the net, but I assume the C3 info is accurate. That came from the C3 website........ follow the link FAQ/Resources, and click on the drop down tab "for medical professionals".

Week 9

Continue to do nothing but follow the golden rule, drinking units pretty much stuck but I'm not concerned, I did browse a few posts and where folk had given details, the number of units start to drop at this point.

Of course Christmas may not be a great time to choose, we'll see?

Continue to be more mindful and I'm still hopeful that things are changing. Had some good support in my battle with keeping my Doc on board, many thanks, Cheeto, nigeir and all41
