My boyfriend has read the book. I have read the book. He went to a clinic to get his first pills when he first found out about naltrexone. when he got home from the clinic, he had interpreted that the people at the clinic verbally gave him the directions to take the pill within 15 minutes of the first drink. he was drunk for a month and couldn't get out of bed. He started taking the pill successfully one hour prior to his first drink. That was great. He was needing to refill the prescription and had received some pills online. On Thursday of last week, he was drunk again, and has been since. He goes through a fifth of Vodka or more a day. We argue about the amount he is drinking. He says I am "not trusting the process". I said he is supposed to wait an hour before his first drink. He says it doesn't matter because he is drinking 24 hours per day. He says he is only to have one pill per 24 hours and it doesn't matter when, just one pill per day. His original prescription bottle states "one pill daily". Someone please clear this up so that we can stop fighting, and he can stop abusing his body. I am so scared for his liver. He is not able to function, to work, or to care for his own body.

1 Hour Before is correct. I actually try to take it 1.5 hours before, just to help me not forget, as I often do with everything in life. Time flies by sometimes.

1 pill a day is the protocol, on drinking days. Some may suggest a 2nd pill for prolonged drinking events. I have done that. Maybe after 4 hours of drinking, pop another pill.

He doesn't have an end to his drinking day. He drinks round the clock. Sometimes he drinks a shot each half hour, sometimes he will take a shot an hour apart. He doesn't like to track...i do, but he says I am controlling. So how does he know when an hour before the first drink is? What should be the wait time between the last drink, the pill and the first drink?

If I was going to be drinking all day, I would take my Nal first thing in the morning. Sounds crazy I guess, but I have been in those situations in the past.

An early morning golf tournament.
Duck hunting trip.
Float trip.
Hangover, cleaning up the basement from the night before party.

Those were actually rare events for me Pre-TSM, but did happen, and I really don't think I have had any close to that since starting TSM 7 months ago. 95% of my pre-tsm abuse comes from drinks after work and more at home on weekdays, and starting mid-day on weekends. I still abuse, just less and less drinking on the way home, less and less drinking alone at home, and less and less drinking alone mid-day on the weekends.

So if it were me in his shoes, starting first thing in the morning, I would probably take one upon waking up and start out taking my 2nd 12 hours after the first one (or roughly mid-day, between waking up and passing out). After that, someone smarter than me regarding safe levels of Naltrexone, needs to speak up on here. I know I've read someone post about 3 a day, 150mg total.

I do know that with any meds, and especially for alcohol and liver abusers like us, it is important to see your Doc and get bloodwork that offers test results for liver functions.

I get blood work once or twice a year, per my Doc, because of my High Blood Pressure and High Cholesterol meds. He doesn't know YET, that I am doing TSM and taking Nal. That day will come hopefully soon.

If he wakes up and can't wait for his first drink, take the Nal first thing. If he MUST, take the Nal with his first drink with breakfast, and not wait the 1 hour. But waiting that 1 Hour makes a ton of difference if you can get him to take it. The 1 hour is referred to as the GOLDEN RULE because thats about how long it takes to digest the Nal and start blocking the endorphins to the brain.

Does he just drink vodka or is also a beer guy? I was both. Pint of vodka on the way home, beer in front of my wife and kids at home, while sneaking supplemental vodka shots at home also. I have given up the vodka cocktails and shot sneaking, and went strictly to beer. The vodka has too much potential to get me drunk really fast and fall into the booze guzzling rut, Nal or no Nal.

However, the official TSM protocol is Take the pill 1 hour before and drink just like you normally would. That being said, per protocol, he probably should start out taking 1 hour before his first vodka shot and continue like normal, with vodka, I guess. It took me about 3 months in to finally give up the vodka and start going the beer route.

He only drinks vodka. At least a fifth per day.
