I don't understand exactly what you are saying here but I take Naltrexone an hour before I am going to drink daily and do aerobics in the a.m. regardless. Let's say I take Naltrexone on Monday at 3:00 p.m. to drink at 4:00 p.m. I would then go to the gym in the a.m. at 8:00 for aerobics with no problems at all. You may not get a runners high, but not sure because there has been enough time lapse so as to have most of the Naltrexone out of one's system. Why not wait til after the run to take it if the run means that much to you? Just a thought!

I bike, run, or swim 3-4 times a week at an intense aerobic level for about 30 minutes. The times of day I do so vary with my work/play schedule -- sometimes when I get up in the morning, sometimes mid-day, or (occasionally) after work before I drink. I also take 25mg naltrexone 1/2 to 1 hour before evening drinking (see my post re: duration of naltrexone in opioid receptors under Questions for Dr. Eskapa, which confirms a 25mg rather than 50mg dose is fine if that works for you). Like you, I'm an everyday, evening drinker, and physically fit. I'm now in my 9th week on the TSM program (BTW -- I'm thrilled with my progress), and during that entire time, I've had a total of 2 "wash-out" days -- where I didn't notice any difference whatsoever in my subjective experience of exercise. Perhaps that has to do with the fact that I don't run marathons (noted for eliciting runner's high) -- I only work-out for short durations at high intensity. That being said -- based on a sample size of 1, and the similarity in our drinking patterns, I'd say -- if you want to exercise, just do it!

Nice to know someone appreciates my icon! We appear to have even more in common than running 5Ks in our late 40's. Yes, I'm a K-12 math/biz ed teacher (who's currently working in a much higher paying position outside the education field -- turns out an M.Ed. and MBA pays better in the private sector).

Thanks for the tread! I have not started TSM yet but had worried about the exercising also. At 0us per day at the moment. I waiting for the OK from my VA docs. My pre was 150us a month. Yes I went cold turkey. Yes I have cravings. I would say about an7 or an 8 but i am distracting myself with as many other things as I can.

To be safe - be off naltrexone at least 36 hours before exercise. That way you are sure to get your good endorphin reinforcement: 48 hours would be better.... hence try weekend washouts with no naltrexone, no drinking but lots of positive endorphin behaviours - exercise, food, sex.... see selective extinction the the book.

Did you read what you just wrote to people who are extremely into exercise, and that is a very integral part of their life?

I get confused with your answers to our questions but still appreciate your input and the time you take to contribute.


I continue to do my "sports" with no lack of endorphin spirit uplifting.


Not to be sassy, I do think we need to seriously re address this issue.

p.s. did not make it down to 35 units but I think I did not go over 60!

ART -- I totally concur with your assessment! If I were to follow Dr. Eskapa's recommendations regarding exercise, I wouldn't have worked out once in the last 10 weeks (as opposed to 4X per week, which I happily continue to do without any reduction in my subjective enjoyment level -- in fact, I'm enjoying exercise more than ever now, 'cause I feel more energetic when I wake up in the morning, compared to pre-TSM). I guarantee following the good doctor's advice in this respect would've resulted in major depression and an escalation in my desire to imbibe!
