I know this is all so overwhelming at first, but you have come to the right place. I can't say I understand all the medical issues you're facing, but it sounds like you're under a doctor's care. Hopefully you trust the medical professional(s) you're working with. If you've been given the OK to take Nalrexone, then you can just keep going with TSM (The Sinclair Method), but make sure it won't interfere with procedures, etc. - (tell all your doctors you are taking Nal before undergoing any procedures). You might also experience some side effects at first like nausea, headaches, and fatigue, but they typically fade away before long. Personally, I didn't have any trouble at all, but everyone is different.
Many have been through AA, detox, etc. without success, and that's what brought them here. The most frustrating part about TSM is that it can take a LONG time to really work. Dr. Eskapa's book says the typical timeframe is 3-4 months, but it can take up to 6 months - maybe even longer for some. You HAVE to be patient, but the reward will be worth the wait.  Don't expect a smooth ride - there will be ups and downs, but that's part of the process. Everyone here will help you along the way. Just follow the rules according to the book: Naltrexone one hour before drinking EVERY time - NEVER drink without Nal, or you could take a step backwards in your recovery.
