 Post subject: Dosage Discussion (sorry wrong place!!)

Hi all. Anyone out there who has upped dosage to .75 or 100 and had success? Or upped and found no difference? I put this out there because I think in Dr. Escapa's book he says .50 should be as much as needed.

Is there value in upping doseage? Does one get 'more coverage' or 'deeper coverage' or what??? I'd be interested to hear of others doing this, including any increase in side-effects.

I have some experience in lowering dosage. I found .25 did not do the job as well as .50

50 mg works fine for me so I've never been tempted to up the dosage. I suppose 25mg would probably be sufficient if you were drinking over a short period of time: say taking the tablet an hour before a 3 hour drinking session. If I was stuck and needed to preserve my NAL I might try that. However, I don't think that is a great idea when you are trying extinction. You want to make sure you are fully covered and not getting any reinforcement. A larger dose is probably safer than a smaller dose.

I would also think that body size could be a factor. You would that think a 100 lb woman would not require the same dose as a 225 lb man, for instance.

"The onset of opiate antagonism following oral administration of naltrexone has been reported to be 15–30 minutes in a limited number of patients who had been receiving morphine chronically. Administration of a single 15-mg oral dose of naltrexone hydrochloride immediately following a single 30-mg subcutaneous dose of morphine has been reported to produce opiate antagonism that is prominent within 6 hours, maximal within 12 hours, and persists for at least 24 hours. The extent and duration of antagonist activity of naltrexone appear to be directly related to plasma and tissue concentrations of the drug. Plasma naltrexone concentrations of 2 ng/mL have been reported to be associated with an 87% blockade of the pharmacologic effects of a 25-mg IV dose of heroin. In one study in former opiate-dependent individuals receiving 100 mg of naltrexone hydrochloride daily and subsequently challenged with a 25-mg IV dose of heroin, the extent of blockade of the effects of heroin was 96, 87, and 47% at 24, 48, and 72 hours after naltrexone, respectively; corresponding plasma naltrexone concentrations were 2.4, 2, and 1.7 ng/mL, respectively.

The duration of the opiate antagonist activity of naltrexone appears to be dose dependent and is longer than that of equipotent doses of naloxone. A single 50-mg oral dose of naltrexone hydrochloride effectively antagonizes the pharmacologic effects of 25 mg of IV heroin or subcutaneous morphine for up to 24 hours. Increasing the dose of naltrexone hydrochloride to 100 or 150 mg reportedly antagonizes the effects of 25 mg of IV heroin for up to 48 or 72 hours, respectively. "

BGH,

My doc suggested that I do 100 mg. He said that he's recommended this for a number of patients and found it helpful. I've noticed that it makes a real difference in keeping me from over indulging. The hangovers suck with heavy headaches but I figure that is part of the cure, right? I know both Yogagirl and Joe12pack are taking 100 mg. Great topic. BTDT

I'm also curious as to how often to take the Nal? For example, I take Nal an hour before drinking. But let's say I continue drinking for the whole day. Does that one 50mg dose of Nal cover me for the entire 24 hour period?
