The protocol in the book is to start with 25mg for a couple days. Many people who started right in at 50 regretted it (but lived to tell the tale and warn others to follow the recommended path).
I started at 25 and after stepping up to 50 had noticeable side effects that included nausea and passing cramps (sorta like gas cramps). I was 'on stage' a lot that week and wanted to minimize my discomfort, so I throttled back to 25mg for the first week. I was fine at 50 after that. It just took me about a week to get acclimated, rather than the two days suggested in the book.
Now that I am cured, on the couple times a month I bother to take a Nal so I can have a glass of wine or a beer or whatever, I will typically have a minor passing cramp (again, more like gas than anything) the following day. I never seem to have side effects while actually *on* the Nal.
