Thanks for the input guys. But Baclofen seems a potentially hazardous med to me, not necessarily for most people who may do well with it, but possibly for my brother. I read a bit about it and what concerns me is its potential for hypotension. My brother has had bouts with very high blood pressure over the last few months and his BP meds were no longer effective. His doc prescribed a more powerful BP med which brought it down but unfortunately with big fluctuations from very low to high readings. I'm concerned about the interaction of high doses of Baclofen with his new BP med. I'm no doctor but still I see enough to warrant caution. I'm in the Middle-east where we do not have specialists in addiction or alcoholism treatments, so reliable medical supervision for something like this is not easy to come by.

We're still hanging on with some hope that the Nal may work eventually. At times (rarely as of late) he may feel no craving for a drink, so its things like that what we're pinning our hopes on. He has had quite a bit of anxiety and stress over the last few months and we're thinking that that may be a factor in why TSM is not showing better results. And we know that TSM worked well for a while (a few months ago), so why not again? Could his first batch of Nal been good and the next one not? Who knows.

p.s to nemo: he is around 160-170 lbs.
to houtx: I'm his brother, not sister. :D

I'm hoping for a breakthrough for your brother and some peace for you as well, he's very fortunate to have a brother like you by his side.

So he began taking taking 2 x 50mg doses (one every 12 hours) to covers his day/night drinking...After about a week or 10 days of doing that, he called me up from Bali Indonesia and told me that the TSM had kicked in. In fact it kicked in so strongly that he lost all cravings and stopped drinking.

Facinating. I was just reading a journal article online this morning regarding the COMBINE Study and noticed that they used a 100 mg dosage:
