I just went through this and posted it but agree with everyone.
I had surgery and was prescribed opiates....I quit NAL 3 days I think before surgery.
I was taking 10 mg of oxycodone and it didn't seem to do much but then again I was later told that was a small dose as I don't know pills just booze. But I was thinking that maybe NAL was effecting the receptors for longer than the 2 days after wards. By the time 5 days had passed I really didn't even need the meds,...I think I stopped after 3 days....
There was a comment maybe by Lena talking about how NAL possibly affects opiates differently than what we're using it for, blockage of endorphins for alcohol.
I hope that helps. For me in the future, I will stop NAL one week ahead of surgery and hopefully the next time I won't be addicted so that won't even be a problem. Either way though, my fear that I would go off the deep end for a short moment off NAL was unfounded in my case.

PS I did not lose ground on my TSM. I drank without NAL for a week. That being said, I was glad to be back on NAL. I didn't improve but neither did I backslide.
