Jerry Seinfeld had a comedy routine where he talked about Day Guy and Night Guy. Night Guy was the one who goes out drinking. Day Guy has to deal with the hangover. Night Guy does not seem to care too much about Day Guy's welfare and is always screwing over Day Guy who rightly feels unjustly taken advantage of by Night Guy.

Before TSM, Day Guy had no real defense against Night Guy. Once I start drinking, Night Guy takes over. Day Guy has to hope that Night Guy will be responsible and not leave a huge mess for him to clean up in the morning. TSM is a weapon that Day Guy can use to fight back against Night Guy. Day Guy is clearly the one who makes the appointment to get the prescription for Naltrexone. It is Day Guy who takes the Naltrexone faithfully one hour prior to drinking.
