I'm in about my sixth pass through AA, doing better than previously despite some issues with their program, but what I REALLY want is STM. Problem is, I took NAL for several months awhile ago while still drinking, and it had no real effect on my drinking behavior. Took 50mg every morning, still drank excessively and inappropriately (secretive, solitary, gulping, behind the wheel, etc). I'm now almost three months completely AF, but would like to start STM in another month or two and see how it goes. My goal would be moderate drinking, maybe 2-3 drinks a couple days a week, but obviously I need to do something different than the last time I took NAL. Any thoughts on why my first NAL experience was unsuccessful, and what I should do this time to get the positive result I so badly want?

Hiya Elpee and welcome back!

Sounds to me like you still have that pesky craving for alcohol in your brain that will not be quieted no matter how many AA meeting you attend! Chances are since you have been depriving yourself of partaking in the sauce-- you want it even more. That's normal my friend!

How long did you give TSM on your first time around? A committment for a year should do it. There is only one rule to follow: Nal (one hour before drinking)+Al+Patience=Regaining Control.

Had you been taking Nal several hours before drinking? Did you take Nal on days you did not drink? The Sinclair Method is very specific. Given enough time it works for the majority of us. How much time it takes depends on your age and how long you have been drinking alcoholically. It took me nearly a year and I am a 42 year old female who had been boozing heavily for a decade plus.

I wish you great success here. This forum has been my sole source of comfort, support and therapy!

Good luck to you,

Ketchikan1

I was taking my Nal first thing each morning along with blood pressure meds and eye vitamins, and for YEARS there have been very few days when I didn't drink at all, most days I started early and kept it up until early evening, so a pretty steady level of ETOH in my system all day, every day. In TSM terms I was probably in the range of 15-20 units a day (!). Kept that (the Nal part) up for probably 6 to 8 months, then stopped Nal as it didn't seem to be doing anything for me. Still have about a four-month supply if I were to take it daily, though of course that's not my goal.

Actually, right now I'm not having any huge cravings, but would definitely like to be able to enjoy a glass or two of nice German wine or sample my son-in-law's microbrews without totally losing control. Short answer, it would appear I can do total abstinence, but would rather not.

Hmmm that's a tough call LP. There is a small percentage of people for whom TSM doesn't work, and it would be a shame to throw away your abstinence to find that out. Can you remember ANY positive effect that nal had on your drinking? A honeymoon phase? Reduced interest in drinking, even if the numbers stayed the same? Greater clarity, fewer blackouts and negative consequences? If you were able to see any subjective changes in your drinking, OR if you had a initial honeymoon phase, then there's a good chance that TSM and nal can work for you. However if you can't locate either of these things in your past experience...it might be safer to stick with abstinence, I fear :(

In truth I wasn't looking all that hard at the time, hadn't heard of TSM at that time and was expecting something magical, I guess.

To continue (got interrupted before): At that point I was looking for the 100% AF pill, which of course Nal isn't, and so wasn't paying much attention to relatively minor swings in units or craving -- or much of anything else that didn't come in a bottle. As I said initially, I've made it through almost three months AF, with much less of the white knuckle factor than ever before, but the truth is, I still want to be able to drink, a little bit, occasionally. With that constant undertone, even at its current low level, TSM is looking very attractive. My hope that would be that strict adherence to TSM along with my newfound commitment and careful attention to when/where/how/why I choose to drink might work.
