My question is about the interval between drinking sessions within a 24 hour period.

If a person finds their cravings are stongest at 3 am, proceed to take their Nal 1 hour prior, and have their session, but then want to drink again later in the day (late afternoon/early evening)is it wise to have another Nal or best just to really try hard to have only one session per day?

I'd take another dose 1 hour prior to the next session. Since using Nal I've not had the urge to continue, or have another session within 12 hours. I did once have an night out where I consumed alcohol stopping at about 1.30am in the morning, then met up with friends the following afternoon in the pub and had my next drink at approximately 2/2.30pm. This is what I did;

50mg Nal 1 hour before eveing session.

Took 50mg Nal on way to pub, got their early so ordered a light meal and a large glass of water (or was it tea can't remember, it wasn't alcohol anyway). When friends came in I ummed and ahhed hanging onto my non-al drink, finally accepted a drink but was busy talking and paying attention and made a point of drinking as slowly as possible. Only had 2 glasses I think before it was time to leave.

What I'm trying to say is that I also used other techniques to prevent it going into a binge.

In the past 50% of the time I'd continue the next morning, not happened yet with Nal and the instance above was only because I was meeting friends I'd not seen for a long time.

Yeah, I agree with UK. Even though nal officially covers you for 24hours, I've found that it really only covers me for 6-8 hours. Any longer than that and I can feel the cravings coming back if I'm not asleep

The half-life of Naltrexone is somewhere between 4 and 6 hours, this means that the Nal is only half as effective after that time and continues to decrease after that. So I would think that if someone is having 2 drinking sessions in a 24 hour period, it would probably be better to take a second tablet 1 hour before the second session starts. But I'm not a doctor so I don't know for sure.
