Lilavi, TSM works for about 80% (some estimates are as high as 90%). For those that it doesn't work for, half fail because of compliance problems, half adhere to The Golden Rule, but it simply doesn't work for them.
If you stick with The Golden Rule and never drink without the Naltrexone, then you would start drinking less and less. Eventually, after a number of months (3-4 months minimum, often longer than that) you would no longer crave alcohol and will have reached "Pharmacological Extinction". At that point, you may decide to have a few drinks a week or maybe only a few drinks per year, but you don't get there overnight, it takes time and you have to keep to The Golden Rule.
I don't know that your hangovers would be any better with Naltrexone, some people say that they're worse.
So yes, you would stop after a few drinks, but it will take time to get to that point. You have to commit to taking the Naltrexone an hour before every time you drink though, and you must keep that up for the rest of your life. On the days you drink, take the Nal and wait an hour before you drink. On the days you don't drink, don't take the Nal.
