I forgot once after 24hrs - started a glass of wine without taking nal. I could still feel the nal effects - no first drink effect. Not good, but maybe no harm done. Nal takes up to 72 hrs to leave your system.

On the flip side, when I take 50mg on an empty stomach, I feel much more of the blocking effect. One hour before on an empty stomach seems best.

Take nal one hour before you drink, every time you drink.

Regarding missing a dose or taking it late, I look at it like this. Each drinking session is an extinction session that is very gradually conditioning you to unlearn your addiction. You didn't become alcoholic after one session and you will not be cured after one TSM session. Taking the pill late or missing one session is not going to defeat the multiple sessions you have under your belt.

Bottom line: if you miss the pill once or are late once, it will not be of consequence. But we only have one rule: take the pill at least one hour before drinking. Shouldn't be too hard to follow - just carry the pill with you.

Assuming I like to drink from dawn to dusk, should I be taking two 50 mg pills a day? Should I split it? Or just take one 50 in the morning?

Getting into medical advice area here -- not sure how to answer to that one. I have never been a 24/7 drinker and have almost always limited my drinking to a few hours so I knew the 50 mg covered me.

As Reality Turns addresses this issue on her January 6 post on this thread (referring to Dr. Ekapa's response) but the answer is best answered by an MD.
