I read The Cure for Alcoholism by Dr Eskapa. I am very keen to give it a try and I have just purchased a 3-month’s supply of Nalmefene (as Naltrexone is not available for home treatment in the UK).

In chapter 17 of the book (for medical profession) I noticed that one of the eligibility criteria listed: “Patients who are actively drinking are better candidates than ones who have been abstinent for more than a week”.

I have abstained from alcohol since November 2016 to date. The cravings are still very strong but intend to persevere. However, if I do fall off the wagon this time: I have my supply of Nalmefene ready to hand and intend to try the Sinclair Program.

Given the eligibility guidelines in Chapter 17 (i.e “Patients who are actively drinking are better candidates than ones who have been abstinent for more than a week”): does anyone know if I can get straight on to taking Nalmefene and start the program after a period of abstinence? would this negatively impact the program outcome?

Any feedback will be much appreciated. Many thanks.

afaik, this is just fine. I'd suggest you start when you have the next day off, as Selincro can be tougher to get used to than Naltrexone. If you have side effects, they should pass in within the first couple of weeks. Check this from Options Save Lives:

I began TSM with just shy of a year of abstinence. It's my opinion long-term abstinence is advantageous and leads to quicker outcomes providing one approaches TSM correctly. Best thing I've ever done. It gave me my life back. Check out my thread on the other forum. There are others within it who've had success as well.

Joe & Bardo, Thank you very much for your feedback and encouragement. This is very useful and much appreciated. As suggested, I will start on TSM if and when I next fall off the wagon. Best

Just a note. An undercurrent to 'abstinence programs/efforts' is the underlying understanding one _will_ relapse. That said it would be better to do a controlled switch to TSM where one has a plan than go on a bender and say, "I guess it's time to start TSM" because you will have lost a huge advantage in your abstinence. So no matter what you do never reintroduce the endorphins and have that first drink on NAL.
