bottomsup, I'm glad that you've found a clinic and that you can get some extra input and advice from them. I was also going to suggest the just in case idea. Although it's obviously not TSM as we know it, I think that as you're in such a unique position a unique way of taking it may be the answer.
My thinking is this: For me, that first dose of Nal gave me an instant "that was a nice couple of drinks but I really don't want anymore" feeling and I stopped after a couple of drinks. You only get cravings at parties, so if you only take it then maybe you'll get that "nice, but no more" feeling with each dose. That avoids you having to go back to more frequent drinking to work on extinction. I've no idea if it can work like that, but IMHO it's a less risky strategy than going back to drinking to do the full TSM.
You asked about Baclofen, but I can't really give you any info as I've only read bits and pieces about it here. I think there is a website called My Way Out which will have details, but it sounds like you've got a plan now that you're happy with so you won't be needeing it.
