Of course, that's probably the #1 anti-TSM argument put forth by AAers. "No real alcoholic would do it because they would just refuse to take the pill so that they can still get high." An easy solution to the problem is to take the pill when you are NOT having cravings, and then forcing yourself to drink whether you feel like it or not. Theoretically, you probably have to get over this initial hump where you are either taking Nal too early (and not getting good extinction), or not taking it at all. Surely, even the most impulsive person can take a pill, wait an hour, and drink a beer or two when they're not in the mood to drink (again, AA abstainers must look at us like we're the biggest wimps!). Just don't try to do it when you're wound up and ready to get going right now.
Another tip, take the pill at a time that will make it very difficult to drink within that hour (e.g. at the start of an hour long Church service, half-way through a movie, at the beginning of an aerobics class, etc...).
