One month ago I spent a week in detox after being admitted to ICU for a very high BAC level. I have been taking Campral and had been taking Naltrexone as well to reduce cravings. Upon researching and finding TSM I stopped the Naltrexone. I have been abstinent for 1 month because I was told in detox that I could never have a drink again without it being considered a relapse. I very much enjoy red wine and would like to be able to indulge in a glass or two over the holidays. Should I take the pill one hour before consuming alcohol so I can enjoy some wine? Is there anything I should be cognizant of given the fact I have been abstinent for one month?

There has been some research that suggests starting TSM after a period of abstinence helps the extinction process move more quickly.

Taking Naltrexone an hour before you drink is a good idea. For many of the people who try it, it significantly reduces harm related to drinking.

You are on very thin ice given your history and I suggest not rushing into anything.

I can tell you, being someone who also did 30+ days of abstinence and then decided to take Nal and take 1 drink....the Nal doesn't eliminate the craving; if you take one drink there is will power involved. However the Nal does seem to reduce the craving and enjoyment of the one drink. I have been able to stop at one drink thru determination that I'm not going to fall off the wagon again.

Like what I understand about your situation, I was not cured w/ Nal before going on my AF run....so I still have a long way to go (I believe) for Nal to work the extinction process for me....For one reason, I'm only allowing myself to drink 1 glass of wine every week or two.

IF you decide to try a glass of wine be absolutely sure you take 50mg of Nal an hour beforehand and only have one glass. Being a binge drinker, you're on a very slippery slope. My recommendation is to err on the side of caution.

Started TSM Aug 2012. Had some success but over time the Nal SEs were so awful that stopped taking Nal. Managed a 30 day (Sept 2012) and 46 day (Feb/Mar 2013) AF period which also contributed to getting drinking under control.
