Am really eager to start the TSM but am in Ireland so am preparing for all the hurdles this will bring. Turned down by first doc today. I did find this article in the Irish Medical Journal Times: http://www.imt.ie/clinical/2013/08/a-ne ... -harm.html. Am concerned about the reference to Selincro 18mg. I thought the recommended dosage was 50mg....? I hope Ireland is going to get this right!

Does anyone know of tips for getting Nal in the Republic of Ireland. I am already feeling desparate and its only my first day of trying :?

I got boxes of nal from Spain in a chemist through a friend 22 euros box of 28 , I must find another contact there now but if you know anyone travelling or living in Spain its worth a try ,hopefully nalmefene will be available here shortly . Best of luck

Thanks for the tip joker man. I will defo check it out. Had a brainwave last night and ended up ordering from AllDayChemist and then signing up for a Parcel Motel. Parcel Motel gives you a Northern Ireland (ie UK) postal address then they deliver it to a Dublin Parcel Motel (type of post box). Hope it works !!

If it's not already clear from the other responses, Selincro is a different drug. 50mg is the standard dosage for naltrexone.

Hi Melisa1928 - yes, can't wait :-) I thought Selincro was the same at Nalmefene (sister drug to Naltrexone) and was used the same way in TSM? Here is a link: http://en.wikipedia.org/wiki/Nalmefene with a reference to it being made under the trade name Selincro.

Now I am getting confused, should the dosage of Nalmefene and Naltexone be the same, i.e. 50mg?

Here is Dr. Sinclair's own discussion, from his 1991 patent application:

Short version: Start at 2.5mg, go to at least 5mg, a fair number of people need more than 5mg, no clinically relevant side effects up to 300mg.

So 18mg for Selincro could well be right. We want full coverage, so the standard dose won't be the lowest dose some people need.
