Here's a news article that talks about using nal to treat compulsive gambling. Interestingly, a 6 month treatment period leads to a 30% abstinence rate, while a 2 year treatment period leads to an 80% abstinence rate. It seems like whether it's alcohol or gambling, the longer you stick with nal the better chance you have of success.

What's also interesting is the top figure, 80% matches Sinclair's figure for success with alcoholics.

It's plainly obvious to me that there are different levels of addiction. Addiction, as The Cure points out, is a learned behavior brought about by conditioning over time. Common sense tells you that the longer one has been conditioned to react/behave in a certain manner, the longer it will take for extinction to occur. However, a major flaw of the book (and the studies it relies upon), is that this basic issue is never addressed. I've asked about it before to the good doctors and got no response -- presumably because there are no studies done on the subject. But it's pretty clear to me that the longer you have been drinking, the longer it takes for extinction to occur. And the real-world results on this board clearly confirm the theory. The youngsters on this board have extinction occur far more rapidly than the long-term drinkers. And not coincidentally, the 3-4 month estimate "to a cure" cited in the book was based upon the experience of college drinkers, not long-term drinkers like most of us on this board.
